Registered Nurse (RN) - Home Health Wexford

UPMC Home Health is hiring a Full-Time Registered Nurse (RN) to provide skilled nursing care to patients in their homes throughout the Pittsburgh-area communities, including Wexford and surrounding areas. This position offers one-on-one patient care in a home health setting with a focus on building meaningful relationships and maintaining a sustainable pace of nursing practice.

Schedule

• Monday through Friday, 8:00 AM to 4:00 PM
• On-call and weekend rotations required
• Flexible scheduling options: 8- or 10-hour shifts
• Dedicated weekend program with premium pay and benefits available

Responsibilities

• Deliver skilled nursing care to patients in their homes throughout assigned Pittsburgh-area communities
• Develop and manage plans of care in collaboration with physicians and interdisciplinary partners
• Provide patient and family education that promotes independence and recovery
• Complete required documentation including OASIS, visit notes, and orders using a provided laptop
• Contribute to high-quality, patient-centered outcomes while maintaining regulatory standards
• Assignments are based on patient need and proximity to your home

Required Qualifications

• Graduate of an accredited school of professional nursing
• Active RN license in Pennsylvania or compact state eligibility
• Valid driver's license and automotive insurance
• Act 33, Act 34, and OAPSA clearances
• Current licensure either in Pennsylvania or, if applicable, a multistate license issued by a participating Nursing Licensure Compact (NLC) state
• CPR certification based on AHA standards (didactic and skills demonstration components required within 30 days of hire)

• Final title and compensation are based on experience, education, and career ladder placement
• Hires and current employees working on an out-of-state NLC license who later change residency to Pennsylvania will have 60 days upon changing residency to apply for licensure within the state
