Registered Nurse RN (Home Health) West Central Broward County

Actify Home Care is seeking Registered Nurses for immediate placement in home health care across West Central Broward County, including Tamarac, Sunrise, Plantation, Fort Lauderdale, and surrounding areas. This is a 1099 independent contractor position offering per-visit compensation for skilled nursing care delivered to geriatric patients in their homes.

Responsibilities

• Provide high-quality, one-on-one skilled nursing care to patients in their home environment
• Complete Start of Care (SOC) OASIS assessments and other OASIS evaluations (Resumption, Recertification)
• Manage high-tech and complex care visits as assigned
• Conduct routine follow-up visits
• Implement effective plans of care to improve patient outcomes and safety
• Complete clinical notes and assessments efficiently using Kinnser (WellSky) documentation system
• Collaborate with therapists, the Director of Nursing, and office staff to deliver coordinated patient care

Compensation

• Start of Care (SOC) OASIS: $80.00 per visit
• Other OASIS (Resumption, Recertification): $55.00 per visit
• Hi-Tech/Complex Care Visits: $40.00 per visit
• Routine Follow-up Visits: $35.00 per visit
• Direct deposit payment every two weeks based on completed Kinnser visit logs

Required Qualifications

• Active Florida Registered Nurse (RN) license or valid Compact License
• Current CPR/BLS certification
• Valid driver's license and reliable transportation

Preferred Qualifications

• Experience with Kinnser/WellSky documentation system
