Registered Nurse (RN), Home Health Senior Living

BAYADA Home Health Care is seeking a Registered Nurse (RN) to provide skilled nursing and rehabilitative care to clients in their homes across Adams and Franklin Counties, Pennsylvania. You will work as part of a multidisciplinary team, enabling clients to receive high-quality care while remaining at home.

This position is available as Full Time, Part Time, or Per Diem. A $1,500 sign-on bonus is available for candidates with home health experience.

Responsibilities

• Perform comprehensive head-to-toe assessments and identify subtle changes in patient condition
• Provide acute and post-acute care including management of conditions such as CHF, COPD, wound care, IV therapy, and post-surgical care
• Triage and manage urgent clinical situations independently in the home setting
• Perform medication reconciliation, patient teaching, and safety monitoring
• Conduct wound assessment, care, and documentation
• Make real-time clinical decisions without immediate on-site support
• Manage a dynamic visit schedule and prioritize urgent visits
• Complete point-of-care documentation and utilize required technology
• Provide care in varied home environments while maintaining cultural sensitivity and respect for patient autonomy

Required Qualifications

• Current Pennsylvania RN license in good standing
• Graduation from an accredited and approved nursing program
• At least one year of current or recent hands-on medical experience within the past two years with the adult or geriatric population in one of the following settings: acute care, emergency department, home health, hospice, ICU, medical-surgical, sub-acute care, or rehabilitation units

Schedule and Compensation

• Flexible scheduling
• Weekly pay schedule
• Base pay: $55-$65 per point, depending on qualifications and subject to negotiation
