Registered Nurse (RN) Home Health

Scarlet Home Health Care is seeking a Registered Nurse to deliver patient-centered care in the Columbus area and surrounding communities. As a Home Health RN, you will work autonomously while maintaining strong clinical collaboration, managing patient care directly in home settings and coordinating with an interdisciplinary team.

Responsibilities

• Deliver direct patient care in patients' homes, working with physicians to manage medication needs and adjust care plans
• Prevent hospital readmissions through proactive clinical management
• Coordinate and guide LPNs, aides, and therapy staff to deliver comprehensive, individualized care
• Educate and counsel family members on care processes, disease management, and recovery
• Complete OASIS assessments, recertifications, discharges, and records of care (ROCs) within the EMR system
• Participate in team orientation, staff meetings, and continuing education opportunities

Clinical Setting

You will work in patients' homes throughout the Columbus area, providing autonomous care with the support of a clinician-led team.

Schedule

Autonomous scheduling allows you to manage your own patient visit calendar.

Required Qualifications

• Active Ohio RN license
• Current CPR certification
• Valid auto insurance
• Reliable personal vehicle for home visits

Preferred Qualifications

• Previous home health or related nursing experience
• Strong communication and organizational skills
• Proficiency with EMR systems
