Registered Nurse RN- Home Health

PruittHealth @ Home is hiring Registered Nurses (RNs) for Home Health Case Manager positions serving patients in Alamance and Guilford County, North Carolina. In this role, you will provide skilled nursing visits in patients' homes, complete comprehensive assessments and care plans, and collaborate with interdisciplinary care teams to ensure quality patient outcomes.

Responsibilities

• Provide skilled nursing visits in the patient's home
• Complete assessments, start-of-care visits, and develop care plans
• Collaborate with the interdisciplinary care team
• Supervise LPNs and Home Health Aides
• Document care using an iPad with AI assist (OASIS experience a plus)
• Educate patients and families to promote independence
• Ensure continuity, quality, and safety of patient care

Required Qualifications

• Active, unrestricted Registered Nurse (RN) license
• Valid driver's license and reliable transportation
• CPR certification
• One or more years of RN experience (home health or acute care preferred)

Preferred Qualifications

• Experience with aging or adult patient populations
• Strong organizational and time-management skills
• Ability to multitask and prioritize a busy caseload
• Confidence working with technology and electronic documentation systems
• OASIS experience
