Registered Nurse (RN) - Home Health - PRN

Life Care Home Health is seeking a Registered Nurse (RN) to provide skilled nursing care to clients in their homes across Texas, Florida, Nevada, and Georgia. This PRN (as-needed) position offers flexible scheduling while delivering direct patient care, coordinating with interdisciplinary teams, and supporting patients of all ages in their home environments.

Clinical Setting

You will work in patients' homes in various conditions, with possible exposure to bloodborne pathogens, bodily fluids, and infectious diseases. Some exposure to unpleasant weather is possible. This role requires reliable local travel and the ability to work a flexible schedule, including PRN emergency call responsibilities.

Responsibilities

• Administer nursing care for clients in their residence and coordinate care with the interdisciplinary team, clients, families, and referring agencies
• Complete initial and ongoing assessments to identify physical, psychosocial, and environmental needs of home health clients, including OASIS assessments at appropriate time points
• Regularly reevaluate client nursing needs and evaluate outcomes of care to assist in recovery or maintenance of life functions and prevent illness and relapse
• Develop, initiate, and revise the Plan of Care (POC) as necessary to ensure quality and continuity of care; plan for client discharge from services
• Provide effective communication to clients, families, team members, and other healthcare professionals; participate regularly in case conferences
• Apply infection control and universal precaution concepts in coordinating and performing client care activities to protect both clients and staff
• Coordinate services and inform physicians and other personnel of changes in client condition and needs
• Monitor assigned cases to ensure compliance with third-party payer requirements
• Complete appropriate documentation in a timely manner to ensure compliance with company policy
• Supervise, teach, and provide clinical direction to other nursing personnel such as LVN/LPN, HHA, and CNA
• Perform on-call responsibilities and provide on-call services to clients and families as assigned
• Participate in in-service programs and maintain or improve competency
• Meet mandatory continuing education requirements of the agency and licensing board

Required Qualifications

• One year of experience as an RN in a clinical care setting
• Completion of an accredited Registered Nursing Program
• Current RN license in good standing in any applicable state(s)
• Current CPR certification
• Current driver's license in good standing
• Reliable transportation and auto liability insurance
• Ability to read, write, and comprehend English
• Hepatitis profile if required to make visits

Preferred Qualifications

• Home health experience
• Bachelor of Science in Nursing (BSN)

Physical Requirements

• Prolonged standing and walking
• Ability to lift and transfer up to 50 pounds
• Ability to meet deadlines and patient needs, make quick decisions, and allocate resources under stressful conditions
• Ability to meet patient and family individualized psychosocial needs
• Hand-eye coordination and manual dexterity

Schedule and Employment

This is a part-time PRN position offering flexible scheduling. On-going employment requires maintenance of a valid RN license in the state where you practice and a valid CPR certificate at all times. It is your responsibility to renew both before they expire to continue employment.
