Registered Nurse (RN) - Home Health - PRN

HomeFirst Home Healthcare is seeking a Registered Nurse (RN) to provide direct patient care in the home setting throughout Rutherford County and surrounding areas. In this PRN role, you will conduct one-on-one patient assessments, develop and implement plans of care in collaboration with the clinical team, provide patient and family teaching, and maintain regular communication with clinical management. You will be responsible for timely and accurate documentation of all patient interactions.

Clinical Setting

• Home-based care in Rutherford County and surrounding areas
• PRN (as-needed) employment type with flexible scheduling

Responsibilities

• Conduct comprehensive patient assessments in the home setting
• Develop and implement individualized plans of care
• Provide direct patient care and patient/family education
• Collaborate with the interdisciplinary care team
• Maintain regular communication with clinical management
• Complete timely and accurate documentation, including OASIS assessments

Required Qualifications

• Graduate of an accredited Registered Nurse educational program
• Current Registered Nurse (RN) license in the state of practice
• Current CPR certification
• Reliable transportation, valid driver's license, and current automobile insurance
• Excellent verbal, written, organizational, collaboration, multi-tasking, and time-management skills
• Strong initiative, follow-through, process orientation, and attention to detail

Preferred Qualifications

• Clinical home health experience
• OASIS assessment experience

Compensation

• Per visit rates: $55 - $145
• Mileage reimbursement provided
