Registered Nurse RN Home Health (PRN)

Empath Home Health is seeking a PRN (as-needed) Registered Nurse to deliver skilled nursing care to patients in their homes throughout north Hillsborough County, including Lutz, New Tampa, Tampa Palms, Temple Terrace, University, and surrounding areas. This position offers flexible scheduling with approximately 10 or more home health visits per week and includes required participation in weekend and holiday on-call rotations.

Responsibilities

• Perform comprehensive patient assessments, including OASIS assessments at required intervals
• Develop, initiate, and revise individualized plans of care as patient needs evolve
• Deliver skilled nursing services and treatments as outlined in the plan of care
• Counsel and educate patients and families on disease processes, self-care techniques, and prevention strategies
• Collaborate with the interdisciplinary team to ensure seamless, holistic care

Required Qualifications

• Graduate of an approved nursing program
• Active Florida RN license
• Minimum of 1 year of direct patient care experience
• Strong assessment, critical thinking, and patient education skills
• Willingness to participate in weekend and holiday on-call rotations

Preferred Qualifications

• BSN or MSN degree
• Prior home health RN experience
