Registered Nurse (RN) - Home Health

GrandCare Health Services is seeking a Registered Nurse to deliver high-quality home health services in the San Fernando Valley. In this role, you will work with patients recovering from surgery or illness in their home environment, developing individualized care plans and coordinating care with an interdisciplinary team. You will help patients and families navigate recovery while ensuring care is delivered safely, effectively, and compassionately.

This position specializes in post-surgical rehabilitation, orthopedic rehabilitation, home health physical therapy, and neuro rehabilitation. You will have the opportunity to see the full impact of your work as you help patients regain confidence, independence, and quality of life.

Clinical Setting

Home-based care across the San Fernando Valley. You will visit patients in their homes and work closely with them and their caregivers to support positive outcomes through every stage of recovery.

Responsibilities

• Develop and initiate the plan of care and assume responsibility for ongoing interdisciplinary assessment and development of individualized care plans in partnership with the patient, representative, and caregiver(s)
• Initiate proper preventative and rehabilitative nursing procedures and provide services ordered by the physician as indicated in the plan of care
• Provide patient and caregiver education
• Identify discharge planning needs as part of care plan development and implement prior to patient discharge
• Ensure that arrangements for equipment and other necessary items and services are available
• Supervise Licensed Practical Nurses (LPNs), Licensed Vocational Nurses (LVNs), and/or Home Health Aides (HHAs)

Required Qualifications

• Valid California Registered Nurse License
• Minimum 2 years of nursing experience
• Current CPR certification
• Valid driver's license with a personally owned, insured automobile in good working order

Preferred Qualifications

• Home health nursing experience

Schedule

Part-time with flexible scheduling.

Compensation

• Per visit: $70 to $140 per completed visit (varies by visit type)
• Paid drive time
• Mileage reimbursement: $0.72 per mile
• Documentation bonus for timely submission of documentation

Travel Requirements

You must have a valid driver's license and a personally owned automobile that is insured in accordance with state or organization requirements and is in good working order. You will travel to patient homes throughout the San Fernando Valley.
