Parx Home Health Care is seeking a Registered Nurse to provide high-quality, coordinated care to patients in their homes and communities. This role focuses on comprehensive patient assessment, individualized care planning, and collaboration with the interdisciplinary healthcare team to ensure continuity of care and optimal patient outcomes.
Responsibilities
- Conduct comprehensive assessments of patients' health status, including physical, emotional, and social factors
- Develop and implement individualized care plans tailored to each patient's needs and goals
- Provide patient education and support to promote self-management and prevent complications
- Coordinate care with physicians, therapists, social workers, and other healthcare providers
- Administer medications, treatments, and procedures as ordered by the physician or authorized healthcare provider
- Document all patient care activities and communicate patient status and progress to the healthcare team
- Provide emotional support and compassionate care to patients and their families
- Participate in quality improvement activities and initiatives to improve patient outcomes and satisfaction
- Maintain professional knowledge and skills through continuing education and staying current with developments in the field
- Adhere to all applicable laws, regulations, and ethical standards of nursing practice
Required Qualifications
- Current and unrestricted RN license in the state of Florida
- 1-3 years of clinical experience in a healthcare setting, preferably in home health, hospice, or community health
- Strong clinical skills and ability to perform evidence-based nursing practices
- Excellent communication and interpersonal skills with the ability to work effectively with patients, families, and healthcare providers
- Proficient in using electronic health records and other healthcare technologies
- Valid Florida driver's license and reliable transportation with auto insurance
- CPR certification (original card required)
- Physical exam and two-step TB test or chest X-ray (no older than 6 months)
- Level 2 criminal background check
- Yearly mandatory in-service participation
