Mary Washington Healthcare is seeking a Part-Time Registered Nurse for Home Health to provide comprehensive, patient-centered care in patients' homes. As a Level II Competent Registered Nurse, you will assess patient needs, develop and implement individualized care plans, and coordinate with the interdisciplinary healthcare team to achieve desired patient outcomes.
Clinical Setting
This position is based in home health, where you will conduct home visits to provide direct nursing care, patient and family education, and discharge planning. You will work collaboratively with patients, families, and healthcare team members to deliver continuous, comprehensive care.
Responsibilities
- Perform comprehensive assessments and develop, implement, and evaluate individualized plans of care on admission and during subsequent home visits
- Provide appropriate and thorough education to patients and caregivers related to the plan of care
- Direct and coordinate nursing care using the nursing process and interdisciplinary teamwork
- Coordinate involvement of patients, families, and health team members in care planning and implementation
- Communicate and coordinate the plan of care and pertinent information with other healthcare team members
- Serve as case manager when assigned to coordinate multidisciplinary services
- Participate in multidisciplinary team meetings to share information and assist with patient management
- Maintain acceptable productivity standards as determined by agency leadership
- Participate in quality and performance improvement activities
- Participate in staff and administrative meetings
- Assist leadership in defining standards of excellence for patient care
- Participate in orientation of new permanent and temporary staff members
- Act as point of contact on hospital and departmental policies and procedures
- Provide input to the Nurse Manager regarding unit needs
- Perform other duties as assigned
Required Qualifications
- Graduate of an accredited nursing program with experience as a Registered Nurse
- Valid RN license from Virginia or reciprocal compact state
- One year of home health or medical/surgical nursing experience
- AHA BLS Provider CPR certification required at hire
- Valid driver's license and must meet MWHC Driver Suitability criteria
Preferred Qualifications
- Bachelor of Science in Nursing (BSN)
Education
Associate degree in nursing (ASN) is acceptable. Bachelor of Science in Nursing (BSN) is preferred.
Compensation
- ASN: $40.59 - $51.65 per hour
- BSN: $42.32 - $53.86 per hour
Physical and Mental Requirements
This position requires constant walking, standing, and use of arms and hands; frequent bending, stooping, kneeling, and squatting; ability to lift 35 pounds and push/pull 20 pounds. You must have auditory and visual skills including color determination, ability to work in confined spaces, and ability to reach overhead. Mental requirements include critical thinking, analytical skills, multitasking, effective communication, collaboration with multidisciplinary teams, and ability to cope with difficult situations. You must be able to tolerate irregular hours including evenings, nights, and weekends.
Environmental Exposure
This position involves potential exposure to radiation, toxic chemicals, and bloodborne pathogens. You must be able to wear appropriate personal protective equipment.
