Registered Nurse (RN) Home Health - Osceola County, FL

Senior Care Solutions, a Medicare-certified home health agency, is seeking a PRN (Per Diem) Registered Nurse to provide skilled nursing care to patients in their homes throughout Osceola County, Florida. This position offers flexibility and autonomy, allowing you to work independently while building meaningful relationships with patients in the community.

Clinical Setting

You will deliver home-based skilled nursing care in accordance with physician orders, agency policies, and Medicare regulations. Your work will involve traveling throughout the assigned coverage area to provide patient care.

Responsibilities

• Perform Start of Care (SOC), Resumption of Care (ROC), Recertification, Routine, Evaluation, and Discharge visits
• Complete comprehensive patient assessments, including OASIS documentation when applicable
• Develop, implement, and update individualized plans of care
• Administer medications, treatments, wound care, injections, and other skilled nursing services
• Educate patients and caregivers on disease management, medications, safety, and available resources
• Monitor patient progress and communicate changes in condition to physicians and the interdisciplinary team
• Collaborate with Physical Therapists, Occupational Therapists, Speech Therapists, Medical Social Workers, and Home Health Aides to coordinate quality patient care
• Ensure documentation is accurate, complete, and submitted within agency timeframes
• Maintain compliance with Medicare Conditions of Participation, state and federal regulations, and agency policies
• Participate in case conferences and communicate effectively with the clinical team

Required Qualifications

• Current, active Florida Registered Nurse (RN) license
• One or more years of nursing experience
• Current BLS/CPR Certification
• Valid Florida driver's license, reliable transportation, and current auto insurance
• Excellent assessment, communication, and organizational skills
• Ability to work independently and manage a flexible schedule

Preferred Qualifications

• Home Health and Medicare experience
• OASIS experience
• Experience with WellSky/Kinnser software
