Registered Nurse (RN) Home Health - North Miami, FL

Senior Care Solutions, a Medicare-certified home health agency in South Florida, is seeking a PRN (Per Diem) Registered Nurse to provide skilled nursing care to patients in their homes throughout the Aventura and North Miami-Dade area, including Aventura, beach areas, El Portal, and potentially Miami Gardens.

As a PRN Registered Nurse, you will deliver patient-centered care in accordance with physician orders, agency policies, and Medicare regulations. This position requires strong clinical judgment, comprehensive assessment skills, and accurate, timely documentation.

Responsibilities

• Perform Start of Care (SOC), Resumption of Care (ROC), Recertification, Routine, Evaluation, and Discharge visits
• Complete comprehensive patient assessments, including OASIS documentation when applicable
• Develop, implement, and update individualized plans of care
• Administer medications, treatments, wound care, injections, and other skilled nursing services
• Educate patients and caregivers on disease management, medications, safety, and available resources
• Monitor patient progress and communicate changes in condition to physicians and the interdisciplinary team
• Collaborate with Physical Therapists, Occupational Therapists, Speech Therapists, Medical Social Workers, and Home Health Aides to coordinate quality patient care
• Ensure documentation is accurate, complete, and submitted within agency timeframes
• Maintain compliance with Medicare Conditions of Participation, state and federal regulations, and agency policies
• Participate in case conferences and communicate effectively with the clinical team
• Travel throughout the assigned coverage area to provide patient care

Required Qualifications

• Current, active Florida Registered Nurse (RN) license
• Minimum of one year of nursing experience
• Current BLS/CPR Certification
• Valid Florida driver's license, reliable transportation, and current auto insurance
• Excellent assessment, communication, and organizational skills
• Ability to work independently and manage a flexible schedule

Preferred Qualifications

• Home Health and Medicare experience
• OASIS experience
• Experience with WellSky/Kinnser

Travel Requirements

• Travel throughout the assigned coverage area to provide patient care
• Valid driver's license, reliable transportation, and current auto insurance required
