Registered Nurse (RN) - Home Health

This position offers the opportunity to provide comprehensive nursing care to patients in their homes. As a Registered Nurse in Home Health, you will conduct patient assessments, develop individualized care plans, coordinate with the healthcare team, and educate patients and families on their treatment needs. You will work independently and collaboratively to ensure quality care while maintaining compliance with federal, state, and agency regulations.

Responsibilities

• Provide and coordinate patient care using the nursing process and professional nursing standards
• Conduct initial and ongoing assessments of patients' physical, psychosocial, and environmental needs
• Develop, implement, and update individualized plans of care
• Coordinate discharge planning with patients, physicians, caregivers, and other healthcare professionals
• Complete OASIS assessments and required documentation
• Supervise CNAs and coordinate care with other healthcare workers
• Educate patients, families, and caregivers on healthcare needs and treatment plans
• Follow federal, state, and agency regulations
• Maintain a clean, safe environment and protect patient privacy and confidentiality

Required Qualifications

• Active Illinois RN license (copy must be included with submission)
• Home Health experience
• Active BLS certification or willingness to obtain
• Valid driver's license and car insurance
• COVID-19 and flu vaccinations required; declinations will not be accepted

Preferred Qualifications

• OASIS and Start of Care experience
• Strong Med-Surg experience with some Home Health experience may be considered
• Local candidates preferred; candidates must be within reasonable distance of the assigned territory
