Life Care Home Health is seeking a Registered Nurse to provide skilled nursing care to clients in their homes across Texas, Florida, Nevada, and Georgia. You will assess patient needs, develop and implement plans of care, coordinate with interdisciplinary teams, and supervise other nursing personnel including LVNs, LPNs, and CNAs. Clinical Setting
- Patient homes in various conditions

Responsibilities
- Promote and exemplify the company's mission, vision, and values
- Complete initial and ongoing assessments to identify physical, psychosocial, and environmental needs of home health clients, including OASIS assessments
- Regularly reevaluate client nursing needs and evaluate outcomes of care
- Assist clients in recovery or maintenance of life functions and prevent illness and relapse
- Develop, initiate, and revise the Plan of Care as necessary to ensure quality and continuity of care
- Initiate appropriate preventative and rehabilitative nursing procedures
- Plan for client discharge from services
- Provide effective communication to clients, families, team members, and other healthcare professionals
- Participate regularly in case conferences
- Apply infection control and universal precaution concepts to protect clients and staff
- Coordinate services and inform physicians of changes in client condition and needs
- Monitor assigned cases for compliance with third-party payer requirements
- Complete documentation in a timely manner per company policy
- Supervise, teach, and provide clinical direction to LVNs, LPNs, HHAs, and CNAs
- Perform on-call responsibilities and provide on-call services as assigned
- Participate in in-service programs and maintain professional competency
- Meet mandatory continuing education requirements
Required Qualifications
- One year of experience as an RN in a clinical care setting
- Completion of an accredited Registered Nursing Program
- Current RN license in good standing in any applicable state
- Current CPR certification
- Current driver's license in good standing
- Reliable transportation and auto liability insurance
- Ability to read, write, and comprehend English
- Hepatitis profile if required to make visits
Preferred Qualifications
- Home health experience
- Bachelor of Science in Nursing (BSN)

Licensing Requirements
- Ongoing employment requires maintenance of a valid RN license in the state where the nurse practices and a valid CPR certificate at all times. It is the employee's responsibility to renew both before expiration to continue employment.
