Registered Nurse (RN) - Home Health, Infusion, FT, Days

Prisma Health Home Infusion is seeking a full-time Registered Nurse to provide comprehensive nursing care to patients receiving home infusion therapy. In this role, you will assess, develop, implement, and evaluate individualized nursing care plans while coordinating with physicians, interdisciplinary team members, and other agencies involved in patient care.

Responsibilities

• Assess patients and develop, implement, and evaluate nursing care plans appropriate to their infusion therapy needs
• Coordinate care with physicians, other healthcare disciplines, and community agencies
• Document all patient problems, interventions, discharge planning, physician orders, and care plan coordination on admission and throughout the certification period
• Perform nursing assessments and interventions at appropriate intervals, monitoring patient progress and adjusting care based on changes in patient condition
• Maintain clear communication with medical staff and supervisors regarding patient care and agency operations
• Initiate discharge planning to address ongoing patient needs prior to discharge
• Maintain patient confidentiality and manage an acceptable caseload according to agency policy
• Submit all required paperwork and documentation according to agency policy
• Participate in chart audits and quality improvement projects
• Accept assignments outside normal duties when needed to fulfill agency needs
• Maintain the highest level of professionalism and conduct at all times

Required Qualifications

• Associate degree in Nursing
• One year of experience as a Registered Nurse
• Current RN compact/multistate license recognized by the NCSBN Compact State or current RN license in the state where you will be working
• Basic Life Support (BLS) certification
• Valid driver's license with an acceptable motor vehicle record as defined by Prisma Health's Acceptable Motor Vehicle Record (MVR) Chart
• Proof of auto insurance
• Ability to pass driver training

Schedule and Travel Requirements

• Full-time position
• Day shift
• This position requires driving using your personally owned vehicle to visit patients in their homes
• You must have reliable transportation and will be expected to adhere to Prisma Health's Driver Safety Policy and department-specific driving policies
