Commonheart is seeking a Wisconsin-licensed Registered Nurse to join our Home Health team. In this role, you will provide comprehensive nursing care to patients in their homes, including assessment, medication management, care coordination, and patient education. You will work as part of an interdisciplinary team to develop and implement individualized plans of care while advocating for patient choice and maintaining professional standards.
Clinical Setting
This is a home health nursing position serving patients across a one-hour radius service area. You will travel to patient homes using a company-provided vehicle (for 0.6 FTE or higher), with all travel expenses covered.
Responsibilities
- Assess patients and identify nursing needs in accordance with current professional standards
- Manage medications and perform clinical interventions as part of the plan of care
- Coordinate implementation of the plan of care and serve as care manager
- Provide patient and family education and counseling consistent with the care plan
- Maintain complete and timely documentation of patient observations, assessments, and ongoing needs
- Supervise other nursing personnel as assigned
- Lead and direct others in providing quality care
- Maintain effective communication with patients, families, and the interdisciplinary team
- Respond to patient and family calls in a timely manner
- Participate in quality assurance and performance improvement initiatives
- Participate in in-service training and continuing education opportunities
- Practice infection control mitigation at all times
- Maintain confidentiality of patient information
- Perform other duties as assigned
Schedule
- Monday through Friday, 8:00 AM to 4:30 PM
- Available FTE options: 0.6 FTE (24 hours per week), 0.8 FTE (32 hours per week), or 1.0 FTE (40 hours per week)
- Rotating on-call duties as assigned
- Full-time and part-time positions available
Required Qualifications
- Current Wisconsin Registered Nurse license
- Ability to relate well to all individuals
- Ability to maintain and protect patient confidentiality

Preferred Qualifications
- At least 2 years of experience in Medical Surgical, Home Health, Hospice, or Long-Term Care nursing
- Gerontology certification
- Wound Care certification
Compensation and Benefits
- Hourly rate: $40.43 to $47.04, based on experience (higher pay may be considered depending on qualifications)
