Registered Nurse (RN) - Home Health - Full Time

Life Care Home Health is seeking a full-time Registered Nurse to provide skilled nursing care to clients in their homes across Texas, Florida, Nevada, and Georgia. You will administer nursing care, coordinate with interdisciplinary teams, and work directly with patients of all ages in home-based clinical settings.

Responsibilities

• Complete initial and ongoing assessments to identify physical, psychosocial, and environmental needs of home health clients, including OASIS assessments
• Regularly reevaluate client nursing needs and evaluate outcomes of care to assist in recovery, maintenance of life functions, and prevention of illness and relapse
• Develop, initiate, and revise the Plan of Care as necessary to ensure quality and continuity of care; plan for client discharge from services
• Provide effective communication with clients, families, team members, and other healthcare professionals; participate regularly in case conferences
• Apply infection control and universal precaution concepts to protect both clients and staff
• Coordinate services and inform physicians and other personnel of changes in client condition and needs
• Monitor assigned cases to ensure compliance with third-party payer requirements
• Complete documentation in a timely manner to ensure compliance with company policy
• Supervise, teach, and provide clinical direction to other nursing personnel including LVNs, LPNs, HHAs, and CNAs
• Perform on-call responsibilities and provide on-call services to clients and families as assigned
• Participate in in-service programs and maintain professional competency
• Meet mandatory continuing education requirements of the agency and licensing board

Required Qualifications

• One year of experience as an RN in a clinical care setting
• Completion of an accredited Registered Nursing Program
• Current state professional RN license in any applicable state
• Current CPR certification
• Current driver's license in good standing
• Reliable transportation and auto liability insurance
• Ability to read, write, and comprehend English
• Hepatitis profile if required to make visits

Preferred Qualifications

• Home health experience
• Bachelor of Science in Nursing (BSN)

Physical Requirements

Working Conditions

• Works in patients' homes in various conditions
• Possible exposure to bloodborne pathogens, bodily fluids, and infectious diseases
• Some exposure to unpleasant weather
• Requires ability to travel locally and work a flexible schedule
• PRN emergency on-call availability

Licenses and Certifications

• Ongoing employment requires maintenance of a valid RN license in the state where you practice and a valid CPR certificate at all times. It is your responsibility to renew both before they expire to continue employment.
