We are seeking a full-time Registered Nurse to provide nursing care for clients in their homes across multiple coverage areas (77581, 77584, 77588, 77047, 77089, 77422). You will work with patients of all ages, coordinating care with interdisciplinary teams, clients, families, and referring agencies to support recovery, maintain life functions, and prevent illness and relapse.
Responsibilities
- Complete initial and ongoing assessments to identify physical, psychosocial, and environmental needs of home health clients, including OASIS assessments at appropriate time points
- Regularly reevaluate client nursing needs and evaluate outcomes of care
- Develop, initiate, and revise the Plan of Care as necessary to ensure quality and continuity of care; plan for client discharge from services
- Provide effective communication to clients, families, team members, and other healthcare professionals; participate regularly in case conferences
- Apply infection control and universal precaution concepts to protect both clients and staff
- Coordinate services and inform physicians and other personnel of changes in client condition and needs
- Monitor assigned cases to ensure compliance with third-party payer requirements
- Complete appropriate documentation in a timely manner to ensure compliance with company policy
- Supervise, teach, and provide clinical direction to other nursing personnel (LVN/LPN, HHA, CNA, etc.)
- Perform on-call responsibilities and provide on-call services to clients and families as assigned
- Participate in in-service programs and maintain or improve competency
- Meet mandatory continuing education requirements of the agency and licensing board
Required Qualifications
- One year of experience as an RN in a clinical care setting
- Completion of an accredited Registered Nursing Program
- Current driver's license in good standing
- Reliable transportation and auto liability insurance
- Current State professional RN license in any applicable states
- Current CPR certification
- Ability to read, write, and comprehend English
- Hepatitis profile if required to make visits
- Ability to lift and transfer up to 50 lbs
- Prolonged standing and walking capability
- Hand-eye coordination and manual dexterity
- Ability to meet deadlines and patient needs, make quick decisions, and allocate resources under stressful conditions
- Ability to meet patient and family individualized psychosocial needs
- Ability to travel locally and work a flexible schedule
Preferred Qualifications
- Home health nursing experience
- Bachelor of Science in Nursing (BSN)
Licenses and Certifications
- Ongoing employment requires maintenance of a valid RN license in the state where you practice and a valid CPR certificate at all times. It is your responsibility to renew both before they expire to continue employment.
- Current driver's license must be renewed before expiration to continue employment.
