Registered Nurse (RN) Home Health - FT

Elite Home Health is seeking a Registered Nurse (RN) to provide skilled nursing care for patients in their homes throughout the Leesburg and surrounding areas. As a Medicare-certified home health agency, we focus on helping patients recover, maintain independence, and improve their quality of life in the comfort of their own homes.

In this full-time position, you will be responsible for delivering comprehensive skilled nursing care, completing detailed patient assessments, and coordinating services according to physician orders and Medicare requirements.

Responsibilities

• Perform comprehensive and ongoing patient assessments
• Complete Start of Care (SOC), recertification, reassessment, and other required visits
• Complete OASIS assessments accurately and within required timeframes
• Develop, implement, and update individualized plans of care
• Provide skilled nursing services in accordance with physician orders and the plan of care
• Monitor patient condition and identify changes in health or functional status
• Communicate significant changes and patient needs to physicians and the care team
• Coordinate care with physicians, therapists, LPNs, CNAs, and other interdisciplinary team members
• Provide direction and supervision to LPNs and CNAs as applicable
• Complete accurate and timely clinical documentation in the EMR
• Ensure documentation supports medical necessity and regulatory requirements
• Participate in care coordination, team meetings, and required training

Required Qualifications

• Graduate of an accredited School of Nursing (ASN or BSN)
• Current Florida RN license in good standing
• Current CPR certification
• Valid driver's license, reliable transportation, and auto insurance
• Ability to meet all required background and compliance requirements

Preferred Qualifications

• Minimum 1 year of nursing experience
• Home health, hospice, or acute care experience
