Registered Nurse (RN) - Home Health

Elara Caring is seeking a Registered Nurse (RN) to provide skilled home health nursing care to patients recovering in their homes. In this role, you will build meaningful relationships with patients and families while delivering clinical care, coordinating patient caseloads, and working with an interdisciplinary team to help patients remain safe and supported at home.

Clinical Setting

You will provide skilled nursing care in patients' homes within the Chicopee territory. This is a full-time position, Monday through Friday.

Responsibilities

• Complete admission and ongoing assessments to identify patient needs
• Provide skilled nursing care according to physician orders and individualized plans of care
• Manage and coordinate care for your assigned patient caseload
• Monitor patient progress, recognize changes in condition, and take appropriate action
• Teach patients and caregivers how to manage care, support recovery, and maintain safety
• Communicate with physicians and the interdisciplinary team to support continuity of care
• Maintain accurate clinical documentation within regulatory requirements
• Protect patient and staff privacy in accordance with HIPAA

Required Qualifications

• Current Registered Nurse license in the state of practice
• Graduation from an accredited nursing certificate, diploma, associate, or bachelor's degree program
• At least one year of nursing experience in a clinical care setting
• Valid driver's license and acceptable driving record, or reliable transportation between patient homes and assigned work locations
• Ability to frequently lift, push, pull, and support up to 50 pounds, including assisting with patient positioning, transfers, and equipment

• Guaranteed salary plus productivity bonus, with pay based on experience ranging from $88,000 to $97,000
