The opportunity

Job Description

Interim HealthCare of Atlanta is seeking full-time Home Health Registered Nurses to provide care to patients in Dekalb County, Georgia, including Stone Mountain, Lithonia, Decatur, and Ellenwood. In this role, you will deliver home-based care to patients with illnesses, injuries, and chronic diseases, often following discharge from hospitals, rehabilitation facilities, or nursing homes. You will work with a team of physicians, oversee LPNs, CNAs, and aides, and serve as the primary clinical decision-maker for your patients.

Responsibilities

  • Examine patients, complete comprehensive assessments, and document progress
  • Report clinical changes and findings to physicians
  • Educate patients on their plan of care, prescribed medications, therapy, diet, and exercise
  • Coach family members on the patient's plan of care, medication management, and home safety
  • Ensure treatment goals are met and coordinate discharge from services
  • Make clinical decisions based on your educational preparation and nursing experience

Schedule

  • Full-time position with a flexible 4-day work week

Required Qualifications

  • Graduate of an accredited nursing program
  • Active RN license in Georgia
  • Minimum of 2 years of nursing experience in home healthcare (preferred)
  • HCHB charting experience
  • CPR certification
  • Negative PPD screening or chest X-ray
  • Valid driver's license, reliable transportation, and auto insurance
  • Knowledge of state and federal home health regulations
  • Clinical proficiency in nursing specialty
  • Strong communication skills
  • Ability to pass federal and state required criminal and abuse background checks and drug screening
