Registered Nurse RN Home Health

Ascension at Home, together with Compassus, is seeking a Registered Nurse (RN) for Home Health to support patient recovery and enable patients to remain in their homes while spending quality time with loved ones and engaging in activities they enjoy.

In this role, you will provide clinical care, case management, and interdisciplinary team oversight. You will empower patients and families to reach their healthcare goals through education on disease management and support for recovery.

Responsibilities

• Support patient recovery and ability to remain at home
• Provide case management and clinical oversight to the interdisciplinary team
• Educate patients and families on disease management
• Empower patients and families to reach healthcare goals

Required Qualifications

• Active RN license in the state where you work
• Two years of experience as a registered nurse
• Current driver's license
• Current CPR and BCLS certification

Preferred Qualifications

• Home health nursing experience
