Registered Nurse (RN) - Home Health

Comfort Home Care is hiring a full-time Registered Nurse (RN) for the Somerville/Medford area to provide psychiatric and home health nursing care. In this role, you will visit patients in their homes to assess their medical and psychiatric needs, develop and modify individualized care plans, provide patient education, coordinate care with providers and families, and maintain clinical documentation. You will manage your own schedule and build long-term relationships with patients while making clinical decisions that directly impact their ability to remain independent and avoid unnecessary hospitalizations.

Schedule

• Full-time position
• Afternoon/evening weekday shift
• Flexible scheduling

Responsibilities

• Assess patients' current conditions, medical needs, and overall well-being during home visits
• Create and modify care plans based on individual patient needs and changing conditions
• Provide education and support to psychiatric and post-operative patients on medication management, wound care, and other clinical topics
• Communicate and coordinate with healthcare providers, families, and care team members to ensure appropriate care delivery
• Document visit notes and clinical information from home to maintain accurate records for continuity of care

Required Qualifications

• Active Registered Nurse (RN) license in Massachusetts
• One year of nursing experience (home health or psychiatric nursing preferred)
• Reliable transportation for patient visits

• Home health nursing experience
• Psychiatric nursing experience
