Registered Nurse (RN) - Home Health

Comfort Home Care is seeking a full-time Registered Nurse (RN) for home health nursing in the Arlington, Winchester, Lexington, and Waltham area. This position offers a morning weekday shift with flexible scheduling and the opportunity to build long-term relationships with patients while providing compassionate care in their homes.

You will work with psychiatric and home health patients, helping them remain independent and avoid unnecessary hospitalizations. This role emphasizes clinical autonomy, meaningful patient relationships, and work-life balance.

Responsibilities

• Assess patients' current conditions, medical needs, and overall well-being at each visit
• Create and modify individualized care plans based on patient needs and changing conditions
• Provide patient and family education on medication management, wound care, and other clinical topics
• Communicate and coordinate with providers, families, and care teams to ensure appropriate care
• Document visit notes from home to maintain accurate records for continuity of care
• Make independent clinical decisions that support patient independence and health outcomes

Required Qualifications

• Active Registered Nurse (RN) license in Massachusetts
• One year of nursing experience
• Reliable transportation for patient visits

Preferred Qualifications

• Home health nursing experience
• Psychiatric nursing experience
