Registered Nurse (RN)-Home Health

We are seeking an experienced Registered Nurse (RN) for a 20-week contract position providing home health nursing and case management services in Barre, Vermont. In this role, you will deliver individualized, goal-oriented nursing care to patients in their homes according to provider orders and established plans of care. When assigned as the lead discipline, you will also function as a Case Manager, coordinating patient care and supporting continuity of services across the interdisciplinary team.

Responsibilities

• Provide individualized home health nursing care using the nursing process and established physician or provider orders
• Perform patient assessments, develop and implement plans of care, and evaluate patient responses to nursing interventions
• Serve as Case Manager when assigned as the lead discipline and coordinate services across the patient's care team
• Educate patients and caregivers regarding treatments, medications, disease management, safety, and ongoing care needs
• Maintain accurate and timely clinical documentation while coordinating with providers and interdisciplinary team members to support continuity of care

Required Qualifications

• Minimum 1 year of Registered Nurse experience
• Current, active Registered Nurse license; applicable Vermont practice requirements must be met before start
• Current BLS/BCLS certification
• Ability to work Monday through Friday days, participate in approximately every fourth weekend rotation, and travel between patient homes

Preferred Qualifications

• Home Health, Hospice, or Case Management experience
• Homecare Homebase (HCHB) EMR experience

Schedule

• Monday through Friday days with approximately every fourth weekend in rotation

Compensation

• $2,000 to $2,297 per week
