Registered Nurse (RN) Home Health - Central/West Miami, FL

Senior Care Solutions, a Medicare-certified home health agency in South Florida, is seeking a PRN Registered Nurse to provide skilled nursing care to patients in their homes throughout Central and West Miami-Dade, including Central Miami, Hialeah, and Miami Gardens.

You will work independently with patients across the service area, performing comprehensive assessments, developing individualized care plans, and delivering skilled nursing services in accordance with physician orders, agency policies, and Medicare regulations. This role requires strong clinical judgment, excellent assessment skills, and timely, accurate documentation.

Responsibilities

• Perform Start of Care (SOC), Resumption of Care (ROC), Recertification, Routine, Evaluation, and Discharge visits
• Complete comprehensive patient assessments, including OASIS documentation when applicable
• Develop, implement, and update individualized plans of care
• Administer medications, treatments, wound care, injections, and other skilled nursing services
• Educate patients and caregivers on disease management, medications, safety, and available resources
• Monitor patient progress and communicate changes in condition to physicians and the interdisciplinary team
• Collaborate with Physical Therapists, Occupational Therapists, Speech Therapists, Medical Social Workers, and Home Health Aides to coordinate quality patient care
• Ensure documentation is accurate, complete, and submitted within agency timeframes
• Maintain compliance with Medicare Conditions of Participation, state and federal regulations, and agency policies
• Participate in case conferences and communicate effectively with the clinical team
• Travel throughout the assigned coverage area to provide patient care

Required Qualifications

• Current, active Florida Registered Nurse (RN) license
• Minimum of one year of nursing experience
• Current BLS/CPR Certification
• Valid Florida driver's license, reliable transportation, and current auto insurance
• Excellent assessment, communication, and organizational skills
• Ability to work independently and manage a flexible schedule

Preferred Qualifications

• Home Health and Medicare experience
• OASIS experience
• Experience with WellSky/Kinnser

Schedule

• PRN (Per Diem) position with flexible scheduling

Travel Requirements

• Travel throughout Central and West Miami-Dade coverage area required
