Registered Nurse RN (Home Health) Central Palm Beach County

Actify Home Care is seeking Registered Nurses to provide skilled nursing care to geriatric patients in their homes across Palm Beach and Broward counties. This is a 1099 independent contractor position with immediate openings and fast-track credentialing.

Clinical Setting

You will work as a field-based RN providing one-on-one skilled nursing care in patients' homes. Service areas include North Palm Beach County (Jupiter, Palm Beach Gardens, Riviera Beach, Juno Beach), Central Palm Beach County (West Palm Beach, Wellington, Royal Palm Beach, Lake Worth), South Palm Beach County (Boca Raton, Delray Beach, Boynton Beach, Lantana), County Line Bridge Zone (Deerfield Beach, Boca Raton, Parkland, Hillsboro Beach), North Broward County (Deerfield Beach, Hillsboro Beach, Lighthouse Point, Pompano Beach, Parkland, Margate, Coconut Creek, Coral Springs), West Central Broward County (Tamarac, Sunrise, Plantation, Fort Lauderdale), East Central Broward County (Oakland Park, Fort Lauderdale, Wilton Manors), and South Broward County (Hollywood, Pembroke Pines, Miramar, Davie, Weston, Hallandale Beach).

Responsibilities

• Provide high-quality, one-on-one skilled nursing care to geriatric patients in their homes
• Implement effective plans of care to improve patient outcomes and safety
• Complete clinical notes and assessments efficiently using Kinnser (WellSky) electronic health record system
• Collaborate with therapists, the Director of Nursing, and office staff to deliver coordinated patient care

Compensation

This is a per-visit compensation model with the following rates:

• Start of Care (SOC) OASIS: $80.00
• Other OASIS (Resumption, Recertification): $55.00
• Hi-Tech/Complex Care Visits: $40.00
• Routine Follow-up Visits: $35.00

Payment is processed via direct deposit every two weeks based on completed Kinnser visit logs. No invoice tracking is required.

Required Qualifications

• Active Florida Registered Nurse (RN) license or valid Compact License
• Current CPR/BLS certification
• Valid driver's license and reliable transportation

Preferred Qualifications

• Experience with Kinnser/WellSky electronic health record system
