Registered Nurse (RN) - Home Health Case Manager - South East City

This position is a Registered Nurse – Home Health Case Manager serving patients across multiple neighborhoods in the South East City area, including Auburn, Gresham, Brainerd, Princeton Park, Beverly View, Washington Heights, Roseland, Cottage Grove Heights, Pullman, East Beverly, Beverly, and Morgan Park. You will develop and maintain therapeutic relationships with patients and families throughout their healthcare journey, providing individualized, holistic care that addresses both physical and psychosocial needs.

Responsibilities

• Develop and maintain therapeutic relationships with patients and families, demonstrating adaptive communication styles based on individual needs
• Provide individualized care to promote physical and psychosocial comfort
• Assess patient and family readiness and involve them in planning and implementing care
• Facilitate patients' and families' right to receive quality, cost-effective care
• Mobilize appropriate resources in response to situations that could negatively impact patient and family outcomes
• Adapt practice to the latest standards according to evidence-based literature
• Participate in quality and process improvement initiatives
• Participate in efforts to reduce risk and improve patient safety
• Meet regulatory and mandatory requirements
• Assess patient and family readiness to learn and modify educational approaches as necessary
• Seek opportunities to enhance knowledge and competence of department members
• Recognize changes in clinical situations and evaluate and implement changes to the plan of care
• Make sound clinical decisions for patients with complex problems
• Prioritize and organize patient care and ensure follow-through with planned care
• Initiate referrals and discharge planning for assigned patients and coordinate using an interdisciplinary approach
• Apply ethical decision-making in all aspects of care
• Monitor for confidentiality and ensure appropriate business conduct
• Delegate correctly to unlicensed assistive personnel
• Seek resources for disruptive behavior that impedes care delivery
• Participate in unit meetings and activities that improve patient care or administrative systems
• Demonstrate effective communication, feedback, and conflict resolution skills
• Critique own nursing practice and maintain required certifications or Bachelor's Degree
• Earn a minimum of 10 contact hours per year

Schedule

Normal office hours are Monday through Friday, 8:00 AM to 5:00 PM. The position includes weekend rotation availability every fourth weekend and coverage for 2 holidays per year. Rotating after-hours on-call coverage for PRN patient concerns is equitably distributed across the area nursing team. You must be willing to work occasional overtime if needed.

Travel Requirements

You must be able to travel to all areas within the Advocate service area in all extremes of weather on all types of roads. You will make visits to private residences, which may not be accessible to people with disabilities. You must have a valid driver's license issued by the Division of Motor Vehicles and be able to transport objects, patients, and equipment.

Physical Requirements

• Ability to lift up to 35 pounds without assistance
• For patient lifts over 35 pounds or when the patient is unable to assist, patient handling equipment must be used with at least one other associate when available
• Ability to assist with ambulation, including turning and transferring patients up to 200 pounds
• Ability to respond quickly to patient needs and work at a fast pace
• Ability to handle multiple tasks and complete work within short timeframes
• Ability to access all areas within the service area
• Exposure to widely varying social and environmental conditions in patients' neighborhoods and homes
• Exposure to potentially infectious specimens and patients
• Probability of exposure to adverse, hazardous, or unpleasant conditions while caring for the sick

Required Qualifications

• Graduate from an accredited school of nursing
• Current Illinois RN license
• Current CPR certification
• Valid driver's license issued by the Division of Motor Vehicles
• One year of medical-surgical nursing experience
• Demonstrated proficiency for the RN role
• Good communication and interpersonal skills
• Good documentation skills
• Computer skills
• Good time management and organizational skills
• Familiarity with patient care equipment
• Earn 10 continuing education credits annually

Compensation

Sign-on bonus available up to $3,000. You must be actively working in this same position at the time of payout. Current internal team members are not eligible at this time.
