Registered Nurse (RN) Home Health Care

Visiting Nurse Service Westchester is seeking a Registered Nurse for Home Health Care based in Dutchess County. In this role, you will provide quality and safe home health services to patients in their homes. You will assess, plan, implement, evaluate, and coordinate all aspects of patient care in collaboration with patients and interdisciplinary health team members.

Responsibilities

• Assess patient needs and develop individualized care plans
• Implement and evaluate nursing interventions in the home setting
• Coordinate care with other members of the health team
• Respond to the needs of patients and families in home settings
• Utilize industry technology such as tablets when working with patients in the field

Required Qualifications

• Active and current Registered Professional Nurse license in the State of New York
• Minimum of one year of experience as an RN in home healthcare or Medical/Surgical programs such as critical care, ER, wound care, or similar specialties
• Ability to assess and respond to the needs of patients and families in the home setting
• Professional liability insurance in the amount specified by the employer
• Ability to utilize industry technology such as tablets in the field
• Valid driver's license with a personally owned automobile in good working condition available for use in the field

Preferred Qualifications

• Experience with OASIS assessments

Compensation and Benefits

• Salary: $105,456 - $121,992 per year
