Registered Nurse (RN) Home Health Care

Visiting Nurse Service Westchester is seeking a Registered Nurse for home health care based in Dutchess County. In this role, you will assess, plan, implement, evaluate, and coordinate all care delivered to patients in their homes. You will work collaboratively with patients and interdisciplinary health team members to ensure the quality and safe delivery of home health services.

Responsibilities

• Assess patient needs and develop individualized care plans
• Implement and evaluate nursing interventions in the home setting
• Coordinate care with other members of the health team
• Respond to the needs of patients and families in the home environment
• Utilize industry technology such as tablets when in the field with patients

Required Qualifications

• Active and current Registered Professional Nurse license in the State of New York
• Minimum of one year of experience as an RN in home healthcare or in a Medical/Surgical specialty such as critical care, emergency department, wound care, or similar specialties
• Ability to assess and respond to the needs of patients and families in the home setting
• Professional liability insurance in the amount specified by the employer
• Ability to drive with a personally owned automobile in good working condition for use in the field
• Ability to utilize industry technology such as tablets

Preferred Qualifications

• Experience with OASIS assessments

Compensation and Benefits

• Salary: $105,456 - $121,992 per year
