Registered Nurse (RN) - Home Health

GrandCare Health Services is seeking a Registered Nurse to deliver high-quality home health services in the West LA area. In this role, you will work with patients recovering from surgery or illness in their home environments, specializing in post-surgical rehabilitation, orthopedic rehabilitation, and neuro rehabilitation. You will develop individualized care plans, provide direct nursing care, educate patients and caregivers, and coordinate with an interdisciplinary team to support positive patient outcomes.

Clinical Setting

You will provide care in patients' homes across West LA, working with a close-knit team focused on patient outcomes and satisfaction. GrandCare specializes in orthopedics, neurology, and pain management, with clinical support and resources available to help you thrive.

Responsibilities

• Develop and initiate plans of care in partnership with patients, representatives, and caregivers
• Conduct ongoing interdisciplinary assessments and develop individualized care plans
• Initiate preventative and rehabilitative nursing procedures as ordered by the physician
• Provide patient and caregiver education
• Identify and implement discharge planning needs prior to patient discharge
• Ensure arrangements for equipment and necessary services are available
• Supervise Licensed Practical Nurses (LPNs), Licensed Vocational Nurses (LVNs), and Home Health Aides (HHAs)

Required Qualifications

• Valid California Registered Nurse License
• Minimum 1 year of nursing experience with home health experience required
• Current CPR certification
• Valid driver's license with an insured, well-maintained automobile

Schedule

Part-time with flexible scheduling.

Compensation

• Per visit pay: $70 to $140 per completed visit (varies by visit type)
• Paid drive time
• Mileage reimbursement
• Documentation and productivity bonuses
• Annual salary range: $80,000 to $155,000 (based on pay-per-visit model)

Travel Requirements

You must have a personally owned, insured vehicle in good working order and be able to travel throughout the West LA coverage area to visit patients in their homes.
