Registered Nurse (RN) - Home Health

GrandCare Health Services is seeking a Registered Nurse to deliver high-quality home health services in the Mid-Orange County area. In this role, you will work with patients recovering from surgery or illness in their home environments, specializing in post-surgical rehabilitation, orthopedic rehabilitation, and neuro rehabilitation. You will develop individualized care plans, provide direct nursing care, educate patients and caregivers, and coordinate with an interdisciplinary team to support positive patient outcomes.

Clinical Setting

You will provide care in patients' homes across Southern California, working with a close-knit team that specializes in orthopedics, neurology, and pain management. This position offers greater autonomy and flexibility compared to traditional healthcare settings, allowing you to build meaningful one-on-one relationships with patients while supporting their recovery and independence.

Responsibilities

• Develop and initiate the plan of care and assume responsibility for ongoing interdisciplinary assessment and development of individualized care plans in partnership with patients, representatives, and caregivers
• Initiate proper preventative and rehabilitative nursing procedures and provide services ordered by the physician as indicated in the plan of care
• Provide patient and caregiver education
• Identify discharge planning needs as part of care plan development and implement prior to patient discharge
• Ensure arrangements for equipment and other necessary items and services are available
• Supervise Licensed Practical Nurses (LPNs), Licensed Vocational Nurses (LVNs), and/or Home Health Aides (HHAs)

Required Qualifications

• Valid California Registered Nurse License
• Minimum 1 year of nursing experience with home health experience required
• Current CPR certification
• Valid driver's license with a personally owned, insured automobile in good working order

Schedule

Part-time with flexible scheduling.

Compensation

This position uses a pay-per-visit model with the following compensation structure:

• Per visit: $70 to $140 per completed visit (varies by visit type)
• Paid drive time
• Mileage reimbursement
• Documentation and productivity bonuses
• Annual salary range: $80,000 to $155,000 (based on visit volume and type)

Travel Requirements

You must have a personally owned, insured automobile in good working order and be able to travel to patients' homes throughout the coverage area. Mileage reimbursement and drive-time pay are provided.
