Registered Nurse (RN) - Home Health

GrandCare Health Services is seeking a Registered Nurse to deliver high-quality home health services in the San Fernando Valley. In this role, you will work with patients recovering from surgery or illness in their home environment, developing individualized care plans and coordinating care with an interdisciplinary team. You will have the autonomy to make clinical decisions while receiving strong organizational support, and you will build meaningful one-on-one relationships with patients throughout their recovery journey.

This position specializes in post-surgical rehabilitation, orthopedic rehabilitation, home health physical therapy, and neuro rehabilitation. You will see the direct impact of your work as patients regain confidence, independence, and quality of life.

Responsibilities

• Develop and initiate the plan of care and assume responsibility for ongoing interdisciplinary assessment and development of individualized care plans in partnership with patients, representatives, and caregivers
• Initiate proper preventative and rehabilitative nursing procedures and provide services ordered by the physician as indicated in the plan of care
• Provide patient and caregiver education
• Identify discharge planning needs as part of care plan development and implement prior to patient discharge
• Ensure arrangements for equipment and other necessary items and services are available
• Supervise Licensed Practical Nurses (LPNs), Licensed Vocational Nurses (LVNs), and/or Home Health Aides (HHAs)

Required Qualifications

• Valid California Registered Nurse License
• Minimum 1 year of nursing experience with home health experience required
• Current CPR certification
• Valid driver's license with a personally owned, insured automobile in good working order

Schedule

• Part-time with flexible scheduling

Compensation

• Pay per visit model: $80,000-$155,000 annually
• Per visit rate: $70-$140 per completed visit (varies by visit type)
