Advocate Health is seeking a Registered Nurse (RN) for Home Health to manage an assigned patient caseload in the University City area, serving the Concord, Kannapolis, and Salisbury coverage areas. This full-time position focuses on coordinating care, managing patient assessments, and directing a multidisciplinary clinical team to ensure quality outcomes and patient satisfaction in the home setting.
Responsibilities
- Manage daily care for an assigned patient caseload, ensuring timely care and documentation according to established plans of care and physician orders
- Collect and analyze patient health data from home assessments, telephonic contacts, and electronic medical records to identify issues and available resources
- Identify patients at high risk for re-hospitalization, falls, or high resource utilization and coordinate appropriate interventions and case conferences
- Prioritize multi-issue situations based on patient health, welfare, safety, and service effectiveness
- Identify barriers to care resolution (language, literacy, psychological factors, etc.) and follow up with appropriate resources or leadership
- Develop and oversee plans of care that are effectively communicated to all parties involved
- Implement care plans and direct coordination of a multidisciplinary clinical team, including supervision of LPNs and Home Health Aides
- Perform ordered treatments, therapies, and procedures in accordance with State Nurse Practice Act, Home Health Conditions of Participation, JCAHO standards, and agency policies
- Evaluate patient progress toward established goals
Schedule
- Monday through Friday
- Case Manager with on-call rotation
- 40 hours per week
Required Qualifications
- Graduate from an accredited School of Nursing
- Current RN license or temporary license as a Registered Nurse Petitioner in your state of work and residence, or if in a National License Compact (NLC) state, meet licensure requirements in your home state
- BLS certification required per policy guidelines
- Ability to lift and support 35 pounds when handling patients, medical equipment, and supplies
- Ability to walk, stand, sit, reach, stoop, bend, push, and pull
- Speak English in good, understandable terms
- Intact senses of sight, hearing, smell, and touch
- Finger dexterity, critical thinking, and ability to concentrate
- Ability to respond quickly to changes in patient conditions
- Physical Abilities Testing required

- Bachelor of Science in Nursing (BSN)
Compensation and Bonuses
- Pay range: $35.50 - $53.25 per hour
- Sign-on bonus up to $3,000.00 for qualified applicants
