Registered Nurse (RN) - Full‑Time Start of Care Specialist (Home Health)

This full-time position is a Start of Care (SOC) Specialist role in home health nursing. You will conduct initial patient assessments, develop individualized care plans, and educate patients and families to support successful transitions into home health services. This role emphasizes clinical excellence, patient connection, and setting the foundation for positive care outcomes from the first visit.

Responsibilities

• Complete Start of Care visits with clinical excellence and compassionate patient care
• Perform comprehensive assessments that accurately reflect patient needs
• Develop individualized care plans that establish the framework for quality outcomes
• Educate patients and families to ensure they feel informed and supported
• Collaborate with the interdisciplinary care team
• Document patient information in the electronic medical record (EMR)
• Provide direct patient care and support throughout the assessment process

Required Qualifications

• Active RN license in Michigan
• Strong assessment skills and clinical judgment
• Reliable transportation
• Solid communication skills
