Personal Touch Home Care, a certified home health agency with over 50 years of service, is seeking a Registered Nurse to provide skilled home care services to patients in Brooklyn, NY. You will plan, organize, and direct comprehensive home care services for adults, geriatric, and special needs populations, with pediatric experience valued.
Clinical Setting
You will work in patients' homes, providing skilled nursing care and coordinating with physicians, interdisciplinary teams, and community providers. This is a fee-for-service position with visits ranging from initial assessments to follow-up care.
Responsibilities
- Complete comprehensive assessments of patients and families to determine home care needs, including complete physical assessments and medical histories
- Regularly re-evaluate patient nursing needs and revise plans of care as patient status changes
- Develop plans of care with the patient's physician, establishing goals based on nursing diagnoses and incorporating therapeutic, preventive, and rehabilitative nursing actions
- Include patients and families in the care planning process
- Administer medications and treatments as prescribed by the physician
- Provide health care instructions and counseling to patients and families
- Identify and implement discharge planning needs
- Perform high-tech nursing skills including indwelling and straight catheter insertion/removal, nephrostomy care, ostomy care, plural and abdominal catheter drainage care, infusion services, g-tubes, and negative pressure wound therapy
- Apply infection control procedures and safety precautions
- Assess patient physical status and health data during each skilled visit
- Inform physicians of changes in patient condition and obtain clarification of orders for treatment plan revisions
- Prepare clinical notes and update the primary physician at least every 60 days; submit all documentation per agency policy and procedures
- Communicate with physicians regarding patient needs and changes in condition
- Coordinate care with other health-related providers
- Participate in on-call duties as defined by agency policy
- Ensure arrangements for equipment and necessary supplies and services
- Instruct, supervise, and evaluate home health aide and personal care aide services every 14 days or as required by law and regulation
- Instruct, supervise, and evaluate LPN/LVN care at least every 30 days
- Exercise professional management and coordinate care among home, community providers, and pediatric tertiary specialists
Required Qualifications
- Bachelor's degree in Nursing from an accredited college or university
- Current active and unrestricted Registered Nurse (RN) license in New York State
- Pediatric care experience in acute and/or home care environments
- Valid driver's license
- Current certification in Basic Life Support for Health Care Professionals
- Minimum of 1 year of recent acute or home care experience, preferably in an institutional setting
- Excellent observation, verbal, and written communication skills
- Problem-solving skills and basic math skills
- Nursing skills per competency checklist
- Basic knowledge of computer operating systems and software applications for communication and documentation
Preferred Qualifications
- Prior experience working with adults and geriatric populations
- Prior experience with special needs populations
Compensation
- Initial/Skilled Observation Care (SOC): $100 per visit, plus $25 bonus if notes are completed within 24 hours
- Follow-up/Revisit: $50 per visit, plus $25 bonus if notes are completed within 24 hours
- Pay range: $50-$125 per visit depending on visit type
