Registered Nurse (RN) Field Staff Home Health (Per Diem)

Southwoods Health at Home is seeking a Per Diem Registered Nurse to provide high-quality, patient-focused care in home settings. In this field staff role, you will manage an assigned caseload of patients from admission through discharge, ensuring a seamless and personalized healthcare experience.

Responsibilities

• Manage a designated caseload and assess patient needs from start of care through discharge
• Develop individualized care plans tailored to each patient's needs
• Monitor patient status closely and carry out ordered interventions
• Adjust care plans as patient needs evolve
• Participate as a key member of a multidisciplinary team to ensure optimal patient outcomes
• Advocate for patients and communicate progress toward established goals
• Accurately and articulately document all clinical findings, interventions, and patient responses in accordance with regulatory standards
• Ensure all clinical processes maintain compliance with regulatory agencies, including OASIS requirements where applicable
• Keep the multidisciplinary team, patients, and families updated on patient progress
• Utilize clinical resources effectively while considering cost-containment without compromising safety or quality of care

Required Qualifications

• Active Registered Nurse (RN) license in the State of Ohio
• Current AHA BLS certification
• Valid Driver's License and proof of current automobile insurance
• Ability to travel to patients' homes within the assigned area
• Strong critical thinking and problem-solving skills
• Effective communication skills
• Exceptional prioritization and time-management abilities with a high degree of autonomy

Preferred Qualifications

• Bachelor of Science in Nursing (BSN)
• Prior experience in home health care
