The opportunity

Job Description

Baptist Health Home Care of KY is seeking a Registered Nurse Clinical Manager for Home Health to oversee all personnel and patient care services in a home-based setting. In this role, you will ensure quality and safe delivery of home health services while managing compliance with clinical processes, Medicare guidelines, and federal regulations.

Schedule

  • Monday through Friday, 8:15 AM to 5:00 PM

Responsibilities

  • Provide oversight of all personnel and patient care services
  • Ensure Start of Care compliance by the clinical team and monitor assigned Outcome Measures
  • Participate in liaison and administrative on-call rotation and patient staffing as needed to manage after-hours team needs and patient-related issues
  • Process complaint workflow and report staffing trends, neglect, abuse, or theft to the Administrator for resolution and follow-up
  • Complete patient visits as needed and assist with any patient issues
  • Participate in managing costs by monitoring supply expenses, overtime, staffing, bonuses, and visits per episode
  • Select and onboard clinical staff
  • Collaborate with intake to provide daily direction and manage referral volume based on staffing availability
  • Maintain positive working relationships with referral sources including physicians, hospitals, skilled facilities, assisted living facilities, and families
  • Provide oversight for liaison and clinical staff, ensuring timely completion of work and charting
  • Identify education needs for the team
  • Manage on-call schedules, paid time off, call-offs, and conduct daily and weekly productivity reviews to ensure proper staffing coverage
  • Review clinical documentation and maintain current knowledge of Medicare, federal, insurance, company, and hospital guidelines, policies, and protocols

Required Qualifications

  • Current RN or PT license in the state of employment
  • Minimum of three years of experience as an RN or PT in an acute care setting
  • Valid driver's license and auto insurance in your name
  • Capable of meeting all physical demands of the position
