Registered Nurse (RN) Case Manager

Kettering Home Care is seeking a Registered Nurse (RN) Case Manager to provide skilled nursing care in patients' homes under the direction of attending physicians. You will work autonomously, managing your time efficiently while delivering care within the scope of practice outlined by your state. This role offers the opportunity to make a direct impact on patient outcomes in a home-based setting with the support of a dedicated team.

Clinical Setting

• In-home patient care
• Part of the Alternate Solutions Health Network family

Responsibilities

• Provide skilled nursing care as directed by the attending physician
• Manage patient care autonomously and efficiently
• Perform all practices and duties within the scope of practice outlined by the state
• Work collaboratively with the scheduling and patient admissions team

Schedule

• Monday through Friday, 8:15 AM to 5:00 PM
• Evening, weekend, and holiday on-call requirements

Required Qualifications

• Current Registered Nurse (RN) license in the state of employment
• Minimum one year of experience as an RN in an acute care setting
• Valid driver's license and auto insurance in your name
• Ability to meet all physical demands of the role
