Paris Regional is seeking a Registered Nurse (RN) Case Manager to join our 154-bed hospital in Paris, Texas. In this role, you will serve as a patient-care innovator, utilizing specialized plan-of-care interventions to shape exceptional patient journeys and directly impact patient wellbeing through cutting-edge technology and evidence-based practices.

Clinical Setting

Paris Regional is part of Lifepoint Health, a diversified healthcare delivery network with acute care, rehabilitation, and behavioral health facilities across 29 states. You will work within a community-based hospital environment committed to making communities healthier.

Responsibilities

  • Consult with nursing staff and multidisciplinary teams regularly to evaluate patient status and appropriateness of medical care, including admission decisions, length of stay, transfers, and discharge planning
  • Monitor patient and family satisfaction and respond to questions and complaints from patients, family members, and payors regarding care
  • Participate in discharge planning by coordinating patient transfers to other facilities and coordinating community resources
  • Provide discharge education and resource referrals to patients
  • Perform chart reviews to identify actual or potential issues with service delivery, patient outcomes, satisfaction, compliance, cost, and reimbursement

Required Qualifications

  • Current state RN license
  • Associate degree from an accredited nursing school
  • Basic Life Support (BLS) certification required within 30 days of hire
  • ACLS and PALS certifications required within six months of hire

Preferred Qualifications

  • One year of nursing experience
  • ASLS certification
