Registered Nurse RN Case Manager Home Health

OSF HealthCare is seeking a Registered Nurse Case Manager for Home Health to identify, assess, plan, implement, monitor, and evaluate clinical outcomes, service utilization, and resource management for patients on their caseload. You will work with patients across all ages from infancy through geriatric, collaborating with an interdisciplinary team to coordinate services including nursing, therapy, social work, home care aides, respiratory therapy, home medical equipment, and home infusion services. This role requires a thorough understanding of OASIS documentation and plan of care development, with accountability for achieving the best possible patient outcomes and identifying patients at high risk for hospitalization.

Responsibilities

• Identify, assess, plan, implement, monitor, and evaluate clinical outcomes for assigned patients
• Manage service utilization and resource allocation for patient caseloads
• Provide evidence-based, patient-centered care at patients' places of residence
• Coordinate interdisciplinary services and collaborate with providers
• Develop and maintain OASIS documentation and plans of care
• Identify and provide focused oversight of patients at high risk for hospitalization
• Apply critical thinking and decision-making skills to patient care
• Contribute to 5 Star Agency Performance and Value Based Purchasing performance

Required Qualifications

• Completion of a nursing program
• RN license in Illinois or Michigan (depending on state of employment)
• Minimum two years of experience as an RN in a healthcare environment
• Current American Heart Association Healthcare Provider BLS certification prior to start date
• Valid state driver's license with proof of active auto insurance
• Excellent interpersonal and communication skills
• Solid computer skills, including proficiency with Microsoft software
• Strong analytical and problem-solving skills with attention to detail
• Ability to provide own transportation and auto insurance
• Commitment to abiding by all laws, rules, and recommendations for safe driving while on duty

Preferred Qualifications

• Bachelor of Science in Nursing (BSN)
• Experience as an RN in home health
• OASIS certification
• Case management experience
• Electronic Medical Record and Microsoft application basic proficiency

Compensation and Benefits

• Pay range: $37.05 - $54.54 per hour (based on years of licensure)
• $3.25 weekend differential
• External candidates with 5 or more years of direct experience receive 24 hours of frontloaded vacation/holiday time on day 1, in addition to generous paid time off plans
