Registered Nurse (RN) Case Manager – Home Health

Baywood Home Care is seeking a part-time RN Case Manager to coordinate care and support patients in their homes in Fairmont, MN. In this role, you will build meaningful one-on-one relationships with home care patients while maintaining their independence and quality of life. You will work a flexible schedule of up to 20 hours per week, coordinated around your availability and patient needs.

Responsibilities

• Complete patient assessments and develop individualized care plans
• Coordinate services with physicians, therapists, caregivers, and community resources
• Monitor patient progress and adjust care plans as needed
• Educate patients and families on medications, treatment plans, and wellness
• Keep accurate clinical documentation and maintain regulatory compliance

Clinical Setting

You will provide care in patients' homes throughout the Fairmont, MN area. Travel between patient homes is required.

Required Qualifications

• Active Minnesota RN license
• Home health, case management, or transitional care experience
• Strong assessment, communication, and care coordination skills
• Must be at least 21 years old

Schedule and Employment

• Part-time position, up to 20 hours per week
• Flexible scheduling coordinated around your availability and patient needs
• On-call shifts are optional; extra pay is available if you choose to participate

Compensation and Benefits

• Pay: $37.44–$45.00 per hour, based on experience
• Mileage reimbursement and access to a company vehicle for business use
