Mercy Health St. Vincent Medical Center seeks a Registered Nurse for Case Management on a PRN basis. In this role, you will manage patient transitions of care through the hospital and into appropriate post-acute settings. You will work to secure third-party payer approval for services provided and ensure appropriate documentation to support patient status, level of care, and DRG assignment.
Clinical Setting
Mercy Health St. Vincent Medical Center is a regional tertiary care facility serving a 150-mile radius of Northwest Ohio and Southeast Michigan. The medical center is verified as a Level I Adult and Level II Pediatric Trauma Center, designated as a STEMI Center, and verified as a Comprehensive Stroke Center by the Joint Commission. The facility has partnered with Nationwide Children's Hospital of Columbus to operate Nationwide Children's Hospital of Toledo on campus.
Responsibilities
- Manage patient transitions of care from hospital to post-acute settings
- Analyze clinical, financial, and social situations to identify problems and alternative courses of action
- Secure third-party payer approval for services provided
- Ensure appropriate documentation to support patient status, level of care, and DRG assignment
- Guide and direct the healthcare team to assure optimal patient outcomes
- Communicate clearly and concisely with physicians, patients, families, and interdisciplinary team members
- Demonstrate creative problem-solving and critical thinking in case management decisions
- Maintain working knowledge of DRG assignment
Required Qualifications
- Current RN license in the state of Ohio
- Minimum of 3 years of clinical experience
- Current CPR certification
Preferred Qualifications
- Bachelor of Science in Nursing (BSN); if not held at hire, must be obtained within 3 years
- Clinical specialty or case management certification
- Case management experience
- Case management certification; if not held at hire, must be obtained within 3 years of hire date
- ACLS certification
- Other relevant certifications

Schedule
- PRN (as needed) position
- 80 hours per pay period
- Weekend, holiday, and on-call rotation required per department policy
