Job Description
Bradford Hills Nursing & Rehabilitation Center, located in Northeast Pennsylvania, is seeking a Registered Nurse (RN) to provide direct patient care in our skilled nursing and rehabilitation facility. You will work as part of an interdisciplinary healthcare team delivering patient-centered care in a long-term care setting.
Responsibilities
- Provide direct patient care in accordance with established nursing standards and protocols
- Conduct patient assessments, identify care needs, and develop effective plans to address those needs
- Develop and implement individualized care plans in collaboration with the interdisciplinary care team
- Collaborate with physicians, therapists, and other healthcare professionals to ensure seamless transitions and optimal patient outcomes
- Participate in quality improvement initiatives
- Contribute to a positive, respectful, and supportive work environment
Required Qualifications
- Current licensure as a Registered Nurse (RN) in the state of Pennsylvania
- Bachelor's degree in Nursing or related field
