Registered Nurse (RN)

Attend Home Care is seeking a Registered Nurse (RN) Case Manager to deliver skilled nursing care in the home health setting. In this role, you will conduct comprehensive clinical assessments, develop individualized care plans, perform skilled nursing visits, and provide patient and caregiver education to support safe, independent living in patients' homes.

Responsibilities

• Conduct Start of Care (SOC) visits to complete comprehensive clinical assessments and develop physician-directed care plans
• Perform skilled nursing visits and ongoing reassessments to monitor patient condition and adjust care plans as needed
• Provide hands-on skilled nursing care, including wound care, medication management, injections, and chronic disease management
• Deliver patient and caregiver education on diagnoses, medications, and preventive care to reduce hospitalizations
• Collaborate with physicians and interdisciplinary team members to ensure coordinated, high-quality care
• Maintain accurate and timely clinical documentation in compliance with state regulations and agency standards
• Supervise and delegate care to LPNs and Home Health Aides to ensure quality and adherence to the plan of care

Required Qualifications

• Active Registered Nurse (RN) license in the state of practice
• Strong clinical assessment and critical thinking skills
• Knowledge of home health regulations and documentation standards
• Excellent communication and interpersonal skills
• Ability to work independently and manage time effectively

Preferred Qualifications

• Experience in home health, skilled nursing, or a related healthcare setting

Employment Type and Schedule

• Part-Time/Per-Diem with flexible scheduling available

Compensation

• Competitive pay
• PRN: $125 per Start of Care visit, $75 per Reassessment visit, plus mileage reimbursement
