Registered Nurse - PRN Home Health Position (Jefferson County, TN)

All Tennessee Caregivers is seeking a Registered Nurse to provide high-quality home health care to patients throughout Jefferson County, Tennessee. In this PRN position, you will assess patients' health needs, develop and implement individualized care plans, and provide ongoing monitoring and support to ensure optimal patient outcomes.

Responsibilities

• Assess patients' health needs and develop and implement care plans
• Monitor patients' vital signs and report any changes to appropriate staff members
• Communicate with patients and families to provide education and support
• Collaborate with other healthcare professionals to coordinate care and ensure continuity of treatment
• Document patients' progress and outcomes using electronic medical records
• Perform other duties as assigned

Travel Requirements

• Willingness to travel throughout Jefferson County is required

Required Qualifications

• Bachelor's degree in Nursing
• Current state license as a Registered Nurse
• Minimum of 1 year of experience as a Registered Nurse
• Excellent communication and interpersonal skills
• Ability to work collaboratively with other healthcare professionals
• Strong problem-solving and critical thinking skills
