The opportunity

Job Description

All County Home Health & Hospice is seeking a Registered Nurse (PRN) to provide home health nursing services throughout the Greater San Antonio area. In this role, you will deliver direct patient care in patients' homes, conducting comprehensive assessments, developing individualized care plans, and coordinating services to meet the health needs of individuals and families within their communities.

Responsibilities

  • Conduct initial and ongoing assessments of patients and families to determine home care needs, including complete physical assessments and health histories
  • Develop and revise care plans based on nursing diagnoses, incorporating therapeutic, preventive, and rehabilitative nursing actions
  • Involve patients and families in the care planning process
  • Administer medications and treatments as prescribed by the physician
  • Provide health care instructions and counseling to patients and families
  • Identify and implement discharge planning needs
  • Serve as Case Manager when assigned, coordinating care for assigned caseloads
  • Prepare clinical notes and communicate with physicians regarding patient needs and condition changes at least every 60 days
  • Coordinate care with community health resources
  • Instruct, supervise, and evaluate home health aide care every two weeks
  • Participate in on-call duties as defined by organizational policy
  • Arrange for necessary equipment and services

Required Qualifications

  • Current RN license to practice professional nursing in Texas
  • Graduate of a National League for Nursing accredited school of nursing
  • Current CPR certification
  • Valid driver's license with a personally owned, insured vehicle in good working order
  • Ability to work independently with minimal supervision
  • Strong observation, problem-solving, verbal, and written communication skills
  • Ability to organize and prioritize workload independently
  • Physical capability to perform prolonged or considerable walking or standing
  • Ability to lift, position, and transfer patients
  • Ability to lift supplies and equipment
  • Capability to perform reaching, stooping, bending, kneeling, and crouching
  • Adequate visual acuity and hearing to perform required nursing skills

Preferred Qualifications

  • Minimum of two years of nursing experience, with at least one year in public health or home care nursing
