Lehigh Valley Health Network is seeking a Registered Nurse - Patient Navigator to serve as a primary point of contact for patients, family members, and referring physicians. This full-time position is based in the LVPG-L Infusion department and focuses on decreasing fragmentation of care, coordinating services, and serving as a clinical liaison for new and existing patients.
Clinical Setting
Lehigh Valley Health Network is a nationally recognized health system with multiple hospital locations. This position is located at Lehigh Valley Hospital - Cedar Crest, 1501 N Cedar Crest Blvd, Allentown, PA 18104.
Schedule
- Full-time, Monday through Friday
- Day shift: 0800-1630
Responsibilities
- Help patients and family members navigate their medical care by identifying and solving treatment-related problems
- Assess and monitor continuity of care, expedite service delivery, and coordinate care to facilitate patient progress
- Collaborate with the patient's medical team for follow-up and monitor patient progress
- Participate in the development, implementation, and review of quality metrics
- Develop and implement patient education materials
- Identify appropriate indicators to monitor and track patients in conjunction with marketing, patient data advertising campaigns, and patient referral outcomes
- Compile data, track outcomes, and make recommendations for process improvement
- Identify and report quality issues to improve efficiency
- Engage in quality assessment, utilization review, risk management, and infection control
- Function as a clinical liaison for new and existing patients using approved criteria and direct clinical experience
- Provide referrals and support for follow-up care for patients who are uninsured or underinsured
Required Qualifications
- Specialized Diploma in Nursing
- Minimum 2 years of experience working as a Registered Nurse
- Active RN license in Pennsylvania (required upon hire)
- Ability to maintain confidentiality of records and information
- Knowledge of patient education techniques and principles
- Knowledge of community resources and how to access them effectively and efficiently
- Demonstrated skills and knowledge in obesity management theories, practices, and techniques
- Proficiency in Microsoft Office applications
- Skilled in both medical and psycho-social assessment

- Frequent sitting and standing
- Frequent keyboard use
- Patient care providers may be required to perform activities specific to their role, including kneeling, bending, squatting, and performing CPR
