REGISTERED NURSE

Mercer Home Health Care is seeking an experienced Per Diem RN Home Care Nurse to provide direct patient care in the home healthcare setting throughout Mercer County. The RN will work collaboratively with the entire home care team and may have the option to case manage as well. This position includes responsibility for starts of care and admissions, with flexibility in scheduling hours.

Responsibilities

• Provide direct patient care to home healthcare patients
• Interact and collaborate with the interdisciplinary home care team
• Perform starts of care and admissions, including weekend availability
• Optionally case manage patients as needed

Required Qualifications

• Current New Jersey RN License
• Minimum 1 year of home healthcare or equivalent medical-surgical nursing experience
• Excellent clinical skills
• Knowledge of current clinical practices
• Exceptional communication and organizational skills
• Basic computer skills
• Availability for weekend and holiday rotations

Schedule

This is a per diem position with flexible hours. Weekend availability is required for starts of care and admissions.
